Nearly 3,000 errors were identified in British Columbia’s euthanasia programme during 2024, according to an internal report obtained through freedom of information legislation.
The province’s Medical Assistance in Dying (MAiD) Oversight Unit found that 51.9 per cent of cases required follow-up, with 2,807 errors identified across those cases.
There were 4,169 requests for euthanasia during the year, of which 72 per cent were carried out. The report said the errors often resulted from missing forms and other administrative problems.
